Is Your Child Eligible for SOCC-Chicago?
Youth are eligible for SOCC-Chicago if they are:- Enrolled in one of the eight Chicago Public Schools (link to schools, click here), or reside in the catchment area for those schools
- Are between the ages of three and twenty-one
- Are involved with more than one child-serving system
- Have a serious emotional disorder (To read about the signs of a serious emotional disorder, click here.)
Who Can Refer a Child to SOCC?
- Parents
- Youth themselves
- Mental health providers
- Staff from other children serving systems
- Even the child
What Happens When a Child is Referred to SOCC?
When a child is referred to SOCC, they will be contacted by an SOCC staff member, either a CCC or FRD. That person will seek to learn your strengths and the strengths of those around you, assess current needs and stresses and bring together a team of people with the right information to help address the problems your child is having.What Kinds of Services Are Involved?
Service plans are designed uniquely for each family and child and may include group or one-on-one counseling, tutoring, and building relationships with teachers or family members, but most importantly, plans also include supports you sometimes might not think about (such as building a team around the child that includes a pastor, a girl scout leader, or a friend). The wrap team is designed to bring together people whom the child and family know well and whom they trust to help turn things around.
